Spring Sports

The Waterford athletic department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The program saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. 

You may review your data at any time to verify that it is current and you will only be required to sign your forms once per school year (and after any update.) Beginning with the 26-27 school year, parents will be able to register their student-athlete for fall, winter and spring sports simultaneously once registration opens in August 2026. This is especially convenient for multi-sport athletes who are currently required to register each season. An email will be sent to all CLMS families announcing when registration opens.

Register at FinalForms and follow the prompts to create your account, create your students and sign your forms. INFORMATION NEEDED TO REGISTER: It will be helpful to have the following information handy to allow for accurate completion of your online registration: Basic medical history and health information. Insurance company and policy number. Doctor, dentist, and medical specialist contact information & emergency contact information.

Please note: Before your student-athlete’s registration is approved, there must be a current physical examination form signed by a physician on file with the school nurse. Physicals are currently valid for 13 months from the date of the physical. If the student’s physical expires during the season, he/she will not be allowed to practice or compete until a new physical exam form is submitted to the nurse’s office. Without an approved registration and current physical examination form on file, the student-athlete will not be eligible to participate.

Click the link above and locate the PARENT icon and click NEW ACCOUNT.  Type your Name, Date of Birth and Email. Click Register. Check your email for an Account Confirmation Email from the FinalForms Mailman. Once received and opened, click Confirm Your Account. Create your new FinalForms password and click Confirm Account. Click Register Student for your first child.

Registering a Student:  Go to the FinalForms webpage and click LOGIN under the Parent Icon. Locate and click the Add Student button.  Type in the Legal Name and other required information. Then click Create Student.  Click the checkbox for the sport(s) your student plans to participate in. Selections may be changed until the registration deadline.  Complete each form and sign your FULL name (i.e. Jonathan Smith) in the parent signature field on each page. After signing each, click the Submit Form and move on to the next form. When all forms are complete, you will see a “Forms Finished” message.

To register additional students, click My Students and repeat the above steps for each additional student.

To update information, login at any time and click Update Forms to update information for any of your students
